Swallowing Problems Treatment in Nashik

Thyroid Gland

The thyroid gland lies in front of the neck just below Adam’s apple and is butterfly-shaped. The right and left wings (lobes) are joined by a small bridge of thyroid tissue (body) called the isthmus. The two lobes lie on either side of the windpipe. The thyroid gland manufactures hormones, which are chemicals that circulate in the bloodstream and affect the function of the cells and tissues in the body. Thyroid hormones are essential for survival.

What is goitre?
Any enlargement of the thyroid gland is called goitre. When the whole gland is diffusely enlarged it is called a diffuse goitre. If there is a single nodule, it is termed a solitary nodule. The majority of goitres are not cancerous. However, a small proportion of goitres may harbour cancer cells and this is the reason why I recommend investigations for all thyroid swellings (goitre).

Investigation and treatment of thyroid lump
During the initial consultation, we will take a detailed history of your problem and assess you. This is followed by an ultrasound scan, which is performed by an expert radiologist. During the ultrasound scan, a needle may be inserted into the lump to collect a sample of cells, this is called the fine-needle aspiration cytology test (FNAC). These cells are then analyzed under the microscope by the pathologist who will determine the nature of the swelling.

There are other tests, such as a CT scan, MRI scan, or nuclear isotope scan which may be required in some cases. Often no treatment may be necessary for the thyroid lump (goitre) and you will be advised to adopt a ‘watch and wait for policy’. Alternatively, surgery may be advised. This is commonly undertaken when there is a suspicion of cancer, pressure symptoms, uncontrolled overactivity, and cosmetic concerns. The surgery is called thyroidectomy and is discussed briefly below.

Thyroidectomy
There are many different terms used to describe thyroid surgery. Total thyroidectomy implies the removal of all the thyroid glands, whereas in a hemithyroidectomy or lobectomy only part of the gland is removed. You will be advised about the exact procedure you will need based on your problem.

The operation is performed under a general anesthetic, which means you will be asleep throughout the procedure. A skin cut (incision) is made across the midline in the neck over the gland. This cut is usually placed along a crease line, so when it heals it is barely visible. The required part of the entire gland is removed after making sure all important nerves, blood vessels, and parathyroid glands are preserved. At the end of the operation, a drain (plastic tube) is placed through the skin in order to prevent blood collection in the operated area. Most patients usually spend 24hours in hospital after the operation. The drain is removed before leaving the hospital and the stitches are usually removed after 6 days.

What is Thyroid?

The thyroid is a small, butterfly-shaped gland that surrounds the trachea, or windpipe, in the front of the neck. It is essential for the synthesis of hormones that control a number of vital bodily processes, such as growth, energy levels, and metabolism.

When the thyroid doesn’t function properly, it can impact your entire body, leading to various health issues.

What is Thyroidectomy Surgery?

Depending on the condition, thyroidectomy surgery involves either a partial or whole removal of the thyroid gland. The thyroid gland, which is situated just below the Adam's apple at the front of the neck, generates hormones that control energy levels, metabolism, and other essential bodily processes.

Types of Thyroidectomy Surgery:

Total Thyroidectomy:

In cases of severe hyperthyroidism or thyroid cancer, removal of the entire thyroid gland is usually advised.

Lobectomy or Hemithyroidectomy

Removal of one thyroid gland lobe. When a growth or nodule is limited to one side of the gland, this procedure is frequently carried out.

Subtotal Thyroidectomy

Removal of the thyroid gland, with only a little section remaining.

The most appropriate type of surgery will be suggested after your condition has been evaluated by your ENT thyroid specialist.

Why is Thyroidectomy Performed?

Thyroidectomy is recommended for the following conditions:

Thyroid Nodules:

If a large nodule is suspected of being cancerous or if it is producing compressive symptoms.

Goiter:

Enlarged thyroid gland causing swallowing or breathing difficulties.

Thyroid Cancer

The main treatment for thyroid cancer usually involves gland removal.

Hyperthyroidism

Thyroid gland hyperactivity that is severe and unresponsive to other therapies.

Cosmetic Concerns

In cases where an enlarged thyroid gland affects appearance.

What to Expect During Thyroidectomy Surgery

Because the surgery is done under general anaesthesia, you will be unconscious the entire time. Here’s what happens:

  • Incision: A little incision is made across the front of the neck by the surgeon, usually along a natural skin crease to leave the least amount of scarring possible.
  • Removal: The necessary part of the thyroid gland is carefully removed while preserving important nerves, blood vessels, and nearby parathyroid glands.
  • Drain Placement: A drain may be placed temporarily to prevent blood collection in the surgical area.
  • Closure: A dressing is put on and stitches are used to close the wound.

Most patients stay in the hospital for about 24 hours after surgery. Recovery time varies, but most people resume normal activities within a week or two.

Risks and Benefits of Thyroidectomy Surgery

As with any surgery, thyroidectomy surgerysuch as bleeding, infection, or hoarseness due to nerve damage. However, these risks are rare when performed by an experienced ENT thyroid specialist.

The benefits of the surgery include relief from symptoms, reduced risk of cancer spread, and improved quality of life. Your ENT doctor will discuss the risks and benefits with you in detail before the procedure.

Post-Surgery Care and Recovery

Some people may need thyroid hormone replacement treatment for the rest of their lives after having a thyroidectomy, particularly if the entire gland is removed.

Follow-up treatment consists of:

  • Thyroid hormone levels should be regularly monitored.
  • A nutritious diet to aid in healing.
  • A healthy diet to support recovery.
  • Avoiding strenuous activities during the initial recovery period.
